We are a paperless office. Please feel free to complete our registration forms via our online portal prior to your visit.
PAYMENT OPTIONS
We accept most major PPO insurances and will gladly verify your insurance before you arrive. Just call or e-mail us with your dental insurance card information and the primary account holder’s date of birth.
We also accept Visa, MasterCard, and American Express as forms of payment. If you’d like to consider Carecredit for interest-free payments for your dental care, you can check if you’re qualified here.
GLOW MEMBERSHIP
No insurance? No problem. We offer a Glow Membership plan for those that don’t have dental insurance. For $589 or $52/month, it includes:
-Exam, cleaning and appropriate x-rays twice a year
-1 emergency visit with necessary x-rays
-10% discount for all treatments
